Planned Care and Diagnostics

The Planned Care and Diagnostics Group focus on streamlining planned care services, particularly towards meeting the target, which is implemented from December 2008.

The target requires that from the GP referral to the start of treatment should take no longer than 18weeks.

This has created a substantial focus on improved access to diagnostics and ensuring administrative processes are streamlined and supported wherever possible by information technology.

Current Projects

Planned Care

Anticoagulation
Development of a primary care based element to the Salford Anticoagulation service. Transfer has already commenced for 50-60% of appointments and 60-70% of patients from the Hope Anticoagulation clinic to primary care settings.

18 Week
Reviewing processes and implementing initiatives towards meeting the 18 week target, which requires no patient to wait more than 18 weeks from GP referral to the commencement of treatment by 2008.

Tier 2
An on-going project to establish primary care based management of patients who would previously have been directed into hospital care. Services include musculo-skeletal triage; minor surgery, including vasectomy; dermatology and headache.

Day Case
This project is reviewing and increasing the proportion of day case procedures carried out within Salford, to ensure national averages are met or exceeded, within safe practice parameters. This benefits patients by reducing overnight stays and minimising the risk of contracting hospital acquired infections.

Diagnostics

Establish diagnostics in designated Health and Social Care Centres
Development of effective and responsive care models, in a community based setting.
